Treatments
The complete reference for fixing every injury. Treatments are applied to a body part through the patient panel.
๐ฉธ Bleeding
Bleeding has four levels. Each dressing controls up to a certain level, using a bandage on an arterial bleed will not hold.
| Treatment | Item | Controls up to | Time | Fee |
|---|---|---|---|---|
| Bandage | bandage | Level 1 | 3.5s | $40 |
| Gauze Packing | gauze | Level 2 | 4.5s | $60 |
| Pressure Dressing | pressure_dressing | Level 3 | 5.5s | $90 |
| Hemostatic Gauze | hemostatic_gauze | Level 4 | 6s | $140 |
| Tourniquet | tourniquet | Level 4, limbs only | 4s | $120 |
Stop the bleeding first. Always. Nothing else matters if the patient is losing 60 ml of blood every two seconds. A level 4 bleed drains a full 5,000 ml in about three minutes.
Tourniquets
A tourniquet stops any limb bleed instantly, but it is a trade, not a fix:
- It adds 15 pain immediately.
- After 15 minutes it starts damaging the limb.
- Every 5 minutes after that, the limb takes another level of tissue damage, up to level 3.
A tourniquet is a bridge to proper treatment, not the treatment. Dress the wound underneath and remove it (Remove Tourniquet, 3s, no item needed).
Reopening
A bandaged but unstitched wound can reopen when the patient sprints, jumps or fights. Suture it if you want it to stay closed.
๐ช Wounds
| Treatment | Item | For | Time | Fee |
|---|---|---|---|---|
| Clean Wound | antiseptic | Any open wound | 4s | $30 |
| Suture Wound | suture_kit | Deep wounds (level 2+) | 9s | $150 |
Cleaning
Clean every open wound. It costs $30 and 4 seconds, and it is the difference between a patient who walks away and a patient who comes back with sepsis.
A dirty wound becomes infected after about 10 minutes, and gets worse every 15 minutes after that. At level 3 it becomes sepsis, a disease that needs 3 doses of antibiotics and steadily drains the patient's health.
Suturing
Requires:
- The bleeding to be controlled first.
- A wound of level 2 or higher.
A sutured wound heals in 10 minutes and cannot reopen. A merely bandaged one takes 15 minutes and can tear open again.
Suturing is played: you place each stitch yourself. See Surgery & Procedures โ Suturing.
๐ฆ Infection
| Treatment | Item | Clears up to | Time | Fee |
|---|---|---|---|---|
| Antibiotic Cream | antibiotic_cream | Level 2 | 3s | $45 |
| Irrigate Wound | irrigation_kit | Level 3 | 5s | $60 |
Irrigation also cleans the wound at the same time.
If the infection has already become sepsis, treating the wound is not enough, the patient needs 3 doses of antibiotics as well.
๐ฆด Fractures
This is the deepest part of the treatment system.
| Treatment | Item | Where | Time | Fee | Heals it? |
|---|---|---|---|---|---|
| Palpate | - | Any part | 3s | free | No, reveals a suspected fracture |
| Splint | splint | Arms & legs | 7s | $120 | No. Field stabilisation only |
| Pelvic Binder | pelvic_binder | Abdomen (pelvis) | 6s | $150 | No. Slows the bleeding inside |
| Cervical Collar | cervical_collar | Neck | 5s | $120 | Yes, 30 min |
| Apply Cast | cast_kit | Arms & legs | 12s | $400 | Yes, 30 min |
| Remove Cast | - | Arms & legs | 6s | free | - |
The fracture workflow
1. Palpate the limb โ fracture suspected
2. Splint it (played) โ stabilised, impairment reduced to 60%
3. Get them to a hospital
4. X-Ray or CT โ fracture CONFIRMED
5. Apply a cast โ impairment down to 35%, heals in 30 min
6. Remove the cast โ doneA splint never heals a fracture. It only reduces the impairment while you get the patient to a hospital. Only a cast heals a limb fracture, and a cast requires a confirmed fracture (X-Ray or CT) and a hospital location.
Or operate. A confirmed arm, leg or pelvis fracture can be plated at the surgery table instead, it is healed on the spot, no cast and no waiting. See Surgery & Procedures.
Fractures you cannot immobilise
Head and chest fractures cannot be splinted or cast. Once confirmed by a scan, they heal on their own over 40 minutes of rest.
The pelvis
A fracture on the abdomen is a broken pelvis, and it bleeds inside. Put a pelvic binder on it, confirm it on the X-Ray, then plate it at the surgery table, it does not heal on its own. See Trauma & Life Threats.
Cracking a cast
A patient who fires a two-handed weapon with a broken arm has a 10% chance per burst to crack their cast or turn a closed fracture into an open one. Healing restarts from zero.
๐ฅ Burns
| Treatment | Item | Time | Fee |
|---|---|---|---|
| Burn Dressing | burn_dressing | 5s | $90 |
| Excision & Skin Graft | - | 15s | $1,500 |
Once dressed, a burn heals at 20 minutes per level. Also relieves 12 pain.
Skin grafts
A deep burn (level 2 or 3) can be grafted in hospital: the dead tissue is cut away and covered with skin from the thigh. It leaves a dressed level 1 burn and takes one level off any infection.
Requires a hospital, the Surgery permission and an unconscious or sedated patient. It is played, see Surgery & Procedures.
๐ค Bruises
| Treatment | Item | Time | Fee |
|---|---|---|---|
| Ice Pack | ice_pack | 3s | $15 |
Bruises heal on their own at 5 minutes per level. An ice pack removes a level immediately and relieves 8 pain. Patients can use these on themselves.
๐ฉป Internal bleeding
There is no field treatment for internal bleeding. No bandage, no drug, nothing. It is found by a CT or MRI, or at the roadside with the ultrasound, and fixed in hospital.
Get the patient to a hospital. At the surgery table it is repaired through a catheter, see Surgery & Procedures โ Bleed repair. A small bleed into the chest (level 1, once known) also drains out with a chest drain.
๐ซ Chest, airway & pelvis
The life threats have their own items. Full detail: Trauma & Life Threats.
| Treatment | Item | For | Time | Fee | Rank |
|---|---|---|---|---|---|
| Chest Seal | chest_seal | Open chest wound | 4s | $80 | 0 |
| Needle Decompression | decompression_needle | Tension pneumothorax | 5s | $250 | 1 |
| Suction Airway | airway_kit | Water in the airway | 5s | $60 | 0 |
| Intubate | intubation_kit | A closing or blocked airway, unconscious or sedated patients only | 9s | $350 | 1 |
| Chest Drain | - | Collapsed lung, hospital | 12s | $600 | 2 |
| Pericardiocentesis | - | Blood around the heart, hospital | 10s | $800 | 2 |
๐ฎ Played, not a progress bar
The splint, pelvic binder, intubation, chest drain, pericardiocentesis and skin graft are short minigames. 3 mistakes and the attempt fails, and a failed attempt costs something:
| Treatment | A failed attempt |
|---|---|
| Splint | +15 pain |
| Pelvic Binder | +20 pain |
| Intubate | The kit is used up, oxygen saturation โ15 |
| Chest Drain | โ200 ml blood, +20 pain |
| Pericardiocentesis | โ150 ml blood, +15 pain |
| Excision & Skin Graft | โ250 ml blood, +10 pain |
How each one is played: Surgery & Procedures.
๐ง Concussion
No direct treatment. A concussion heals on its own in 15 minutes.
While it is active the patient's vision blurs and they can black out while driving at speed. Tell them not to drive.
Found by CT or MRI.
๐ฃ Pain
Pain is not an injury, it is a total that builds up from every injury the patient has. High pain:
- slows their walking speed,
- shakes their aim,
- can make them stumble while running.
It drains slowly on its own once they are stable, and drops fast with painkillers. See Medications.
