Equipment
๐ The stretcher
Deploy from the stretcher item, or take one out of the back of an ambulance.
Positions
The stretcher has four real positions you can switch between:
| Position | |
|---|---|
| Flat | Normal transport height |
| Low | Lowered, CPR can be performed on a patient here |
| Sitting up | The patient sits up |
| Folded | Collapsed for storage, CPR also possible |
Using it
- Place the stretcher.
- Load the patient (they must be within 3 m).
- Push it, you lean into it and walk normally, wheels on the ground.
- Load it into the back of an ambulance.
Auto-bed: let go of a stretcher within 2 m of a free hospital bed and the patient is moved onto the bed automatically.
CPR while moving
With the stretcher in the low or folded position, a second medic can ride along doing CPR while a colleague pushes. This is how you keep a patient alive on the way to the ambulance.
You cannot push a stretcher with two broken arms.
โฟ The wheelchair
Deploy from the wheelchair item.
- The patient sits in it and can propel themselves at 1.6 m/s.
- Or a medic can push them.
Using the item (or being discharged with one) places the chair right behind you, facing the way you face, so you sit straight down into it facing forward.
The chair model comes from Config.Wheelchair.models in shared/config/equipment.lua. The first model in the list that is streamed on your server as a prop is used. Vehicle models (for example a drivable iak_wheelchair vehicle) are skipped, because a vehicle spawned as a prop has no wheels, so the script falls back to the next model, prop_wheelchair_01 by default. To use a custom chair, stream it as a prop and put its name first in the list.
Given out and equipped automatically when a patient is discharged with the Wheelchair required mobility. Someone with both legs broken needs one, without it they can only crawl. See Mobility Orders.
๐ฆฏ The crutch
The crutch item. The patient walks with a cane animation and cannot sprint, jump, aim or fight while using it.
Given out and equipped automatically with the Crutches required discharge. Someone on crutch orders who walks without it limps slowly and stumbles, see Mobility Orders.
A crutch is refused while both legs are broken, it won't hold you. That needs a wheelchair.
๐ The medical bag
Deploy the medbag item on the ground and it becomes a shared stash.
| Slots | 20 |
| Weight | 30 kg |
| Use radius | 5 m |
This is the single most useful piece of kit on a multi-medic scene. Any item inside the bag can be used for treatments by anyone within 5 m, nobody has to carry everything, and nobody has to pass items back and forth mid-resuscitation.
Comes stocked
A brand-new bag is packed the first time it is deployed:
| Bleeding | 10 bandages, 10 gauze, 5 pressure dressings, 3 hemostatic gauze, 2 tourniquets |
| Wounds & fractures | 3 antiseptic, 2 suture kits, 2 splints, 1 cervical collar |
| Trauma | 2 chest seals, 1 decompression needle, 1 suction & airway kit, 1 IO needle |
| Drugs | 3 morphine, 2 fentanyl, 2 epinephrine, 2 naloxone, 2 saline bags |
| Tools | Pulse oximeter, BP cuff, stethoscope |
Pick the bag up and its contents stay inside, it is the same bag next time, and it is never restocked for free. Change the list in Config.MedBag.startingSupplies (enabled = false for empty bags).
๐ The patient monitor

Deploy the patient_monitor item (rank 1+), then connect a patient to it.
| Live vitals | Continuous heart rate, blood pressure, oxygen, breathing and temperature, with a real ECG trace |
| Not the rest | Lung and heart sounds, the airway, pupils and blood sugar still need a hands-on check or a tool |
| Drawn on the prop | Rendered directly onto the monitor's own screen, visible from 8 m |
| Leads | The patient must stay within 4 m or they disconnect |
| Alarms | Sounds when the patient goes critical, and fires a code blue |
Ambulances have a monitor in the back, so a transported patient can be monitored on the move.
Monitors can also be placed permanently next to hospital beds through the hospital creator.
โฐ๏ธ The body bag
For patients who could not be saved.
| Item | bodybag |
| Minimum dead time | 60 seconds |
| Takes | 10 seconds |
| Police can use it | Yes (can be turned off) |
| Morgue delivery reward | $250 |
Bagged patients can respawn immediately, they are not held hostage by the bag.
Server owners can set different inventory wipe rules for body bag deliveries than for normal respawns, and different rules again per job.
Once bagged, a body can be carried, loaded into a vehicle, and driven to a morgue. See The Morgue.
๐ง Carrying and dragging
| Action | Blocked by |
|---|---|
| Carrying a patient | 1 broken arm |
| Doing CPR | 1 broken arm |
| Dragging a patient | 2 broken arms |
| Pushing a stretcher | 2 broken arms |
Casts and splints count as broken. Your own injuries are not cosmetic.
Running while moving someone
By default medics walk while pushing a stretcher, carrying, escorting or pushing a wheelchair. To allow running, change Config.Sprint in shared/config/equipment.lua (or /emsadmin โ Settings โ Equipment):
Config.Sprint = {
stretcher = false, -- pushing a stretcher
carry = false, -- carrying someone over the shoulder
escort = false, -- escorting / dragging someone
wheelchair = false, -- pushing someone in a wheelchair
}Set any of them to true to allow running for that action.
๐งฐ Treatment props
Treatments leave visible props on the patient's body, a bandage on the forearm, a tourniquet on the thigh, a splint, a cast, a cervical collar, an oxygen mask, an IV line.
Anyone looking at the patient can see them. It is how another medic can tell at a glance that the leg has already been splinted.
Positions can be tuned live on yourself in /emsadmin โ Tools โ Prop positions, pick the prop and body part, line it up, and press ENTER to copy the finished config line.
